Care Manager: An efficient healthcare management system designed to optimize program development processes and simplify the management of patient information.

Background
The team sought a platform for identifying potential members eligible for enrollment in specific programs. This platform should provide flexibility for creating new programs and conducting analyses on various programs within the system, with a key focus on achieving a seamless user experience.
I contributed to design excellence across all facets, taking the lead in UX and UI for critical components of the application side within the platform.

Project Plan
Creating a thorough UX plan means:

Discovery
"After extensive discussions with stakeholders and thorough research, the following key points have been identified as crucial in understanding the goals for care managers:

Creating a plug-and-play environment where each member is pre-identified with their current condition, acuity level, and the likelihood of developing a condition.

Enabling the setup of new programs through a user-friendly interface, allowing the selection and addition of conditions.

Defining programs with specific conditions, a unique set of parameters, accuracy details, exclusions, and model scores.

Allowing care managers to test new programs, assessing how many members fit in and checking for conflicts with existing programs

To guide our Care Manager effectively, we need to make it easier for them to navigate. By understanding what users might want, we've identified four main things they need to do:

Facilitating the seamless creation and viewing of programs.
3.Program Analysis:
Providing tools for in-depth analysis of programs.
4.Program Comparison:
Enabling users to compare various programs effortlessly.
By addressing these key pathways, we aim to optimize the "Care Manager" experience, making it more efficient and intuitive.

Information Architecture
Having established the framework for our system, it became crucial to align with our users' preferences regarding the Dashboard, program creation and viewing, program analysis, and program comparison pages.

To address this, I explored into previous research and findings.
This in-depth investigation unveiled a variety of ways in which users can collaborate within our platform, highlighting the imperative for a well-structured Information Architecture (IA).
Strategic Choices in Crafting Visuals:

The card was useful for displaying the proportional relationship between parts of a whole.
The card plays a crucial role in data analysis by visually highlighting differences, aiding decision-making, and providing a clear overview of relationships and trends within a dataset.


Line graph here, plays a important for displaying trends over time, making comparisons between datasets, and visualizing relationships between variables. They provide a clear and simple way to analyze and interpret data.

A significant undertaking involved refining the Rule Builder to ensure user-friendliness. Through extensive research and discussions with stakeholders, we devised a straightforward and clear design, streamlining the process of creating rules for program development
Final Design
In the final design phase, we translated user insights into tangible solutions through wireframes. These wireframes serve as a visual blueprint, outlining the streamlined user journey, intuitive interactions, and a cohesive interface. Each wireframe meticulously addresses user challenges, fostering a intuitive, accessible and seamless experience across complex data management, program creation, condition identification, and member integration.
